The transcript evaluation request is for individuals who possess a bachelor's degree or higher and are interested in completing a program at Purdue University West Lafayette for an initial teacher, school services, or administrative license or an addition to a license.

If you completed an out-of-state preparatory program and/or hold an instructional license in another state and want to be licensed in Indiana, do not submit this form. Visit https://www.doe.in.gov/licensing/teacher#OutState

Non-U.S. Graduates: if your degree and coursework were completed outside of the United States, you will need to provide an official evaluation of Foreign Studies from a member of the National Association of Credential Evaluation Services.
